What causes the stem to break off?

Over watering, leaving water in the crown, and other contrivances that cause rot.

Here is a link for a PDF on Rhizoctonia, the fungal disease which causes root rot.
    https://www.staugorchidsociety.org/PDF/RhizoctoniabySueBottom.pdf
Sue Bottom has several PDF free downloads on pests and diseases in orchids with good color photographs.
I highly recommend this kind woman's data base on diseases .
    Very helpful for growers.
Sue is American I believe and also has a link for contact in one of the PDF documents.
I also had this happen in a couple of Phals that I tried in a different mix. It stayed too wet and the roots rotted.
If you still have some aerial roots and can treat the disease remove the rot it is possible to save.
Hope you get some benefit from this.
